According to the Customs and Border Protection of the United States, there is no set format for a minor travel consent form. However, it is recommended that the letter answer, at a glance, the following:
Who is traveling
What the travel plans are or what the form is trying to convey
Where the minor is traveling from and to
When the minor will be traveling
Why the minor is traveling (to accompany a parent on a business trip, for vacation, etc.)
Contact information for the parents
